I give permission for my Scout to attend Troop 1299’s Wilderness Survival Campout listed above.
- I understand that my Scout must be at Suncreek United Methodist Church by 5:30 pm on Friday, in a clean scout uniform (including a troop t‐shirt), with all of his gear packed and ready to go for a prompt 6:00pm departure. We will return Sunday at approximately 12:00 PM AT BOON ELEMENTARY. Parents will receive a text 30 min prior to arrival.
- I understand the following points are required in order for my Scout to attend this event.
- RX: I understand I need to provide any prescriptions my Scout will need to take during the event to the acting Adult Medic, in a zip lock bag, in the original container, with specific instructions on exact dosage and frequency (AM/PM, Hourly, etc.).
- Medical devices: I understand a requirement to participate in 1299 activities is for any medical history listed on my medical form, I must physically show the Scoutmaster or the Troop Medic my doctor prescribed “on person” medical safety device, such as an inhaler for asthmatic reactions, or Epi‐pens for allergies, etc.
- I understand this must take place prior to the beginning of the activity and if I do not have the medical device, I will not be allowed to attend the event.
- Scouts are allowed to bring personal snacks and drinks to this event if the driver allows.
- Electronics (iPod, DVD player, games) are NOT permitted on this campout, phones will be left in cars.
- In case of emergency Adult Leaders will have phones that scouts may contact parents with.
- I understand that safety and concern for others and the appropriate conduct as a Scout throughout this event is a top priority and I have discussed this with my Scout. If any problems arise, I may be reached at the number below to be asked to assist in resolving the issue.
- I understand all payments and medical forms must be submitted prior to departure.
